What's Included in the Style #2 Proposal/Change Order Forms Combo Pack Download
These forms all work on both Windows and Macintosh computers. The .doc forms require Microsoft Word; the PDF forms require the free Adobe Reader.The list of documents:
- Proposal and Change Order forms in WORD format (coordinated with each other) - a logo can be added as well as your company name and info
- Proposal and Change Order forms in PDF (coordinated with each other) - you can add your company name and info and if you you'd like your logo in there I can make that happen for you :)
- A document to help with any questions about the proposal forms (plus I'm always available - 365 days per year)
Both versions (WORD and PDF) have the following standard terms and conditions with provision to charge interest:
"All material is guaranteed to be as specified. All work to be completed in a substantial workmanlike manner according to specifications submitted, per standard practices. Any alteration or deviation from above specifications involving extra costs will be executed only upon written orders, and will become an extra charge over and above the estimate. All agreements contingent upon strikes, accidents or delays beyond our control. Owner to carry fire, tornado and other necessary insurance. Our workers are fully covered by Workmen’s Compensation Insurance. Accounts overdue beyond 30 days of billing will be charged at an interest rate of ___% per annum. Customer is also liable for an additional ___% of unpaid balance plus incidental collections costs, including attorney fees. If either party commences legal action to enforce its rights pursuant to this agreement, the prevailing party in said legal action shall be entitled to recover its reasonable attorney’s fees and costs of litigation relating to said legal action, as determined by a court of competent jurisdiction."

If you are required to show your license and expiration date on your proposal form then you'll need one of the following three forms:No matter what you need, standard terms and conditions, or terms with the ability to charge interest, or a blank field to enter your own terms - you're covered.
If You Are Not Required To Include A License # On Your Bids
If you do *not* have to include a license number with your bids then please check out Styles #4, #5, and #6. #5 is equivalent to this Style #2 but without provision for license.If you have questions please do not hesitate to contact me.
