Forms available in two formats

These forms come in two versions, Word and pdf

Included in the package are forms that work with Microsoft Word for Windows and Macintosh and with the free Adobe Reader for Windows and Macintosh.

Please feel free to use either version, or both if you'd like. :)

The difference between the Word version and the pdf version

Both versions will stay on your computer so that you can use them forever.

The main difference between the Word and pdf versions, besides the fact that the Word version works with Word and the pdf version works with the Adobe Acrobat Reader ;), is that you can save filled-in copies of the Word version, whereas the pdf version will only save not-filled-in copies of the form (unless you're using a premium version of the Adobe Acrobat program, rather than just the free Reader).

When you fill in the pdf version you want to be sure to print as many copies of the form as you'll need before you close the form, because once you close it the info you typed in is erased and the form goes back to its "ready-to-be-filled-in" state, ready for the next time you need it.
